Actually, while at the 1994 Popular Culture conference in Chicago, I met a
woman (never got her name) who was "studying" a revitalization movement of
trephynation in California. Of course, they had focused on the
spiritual/ritual (read structural) aspects of it and negated the medical
(functional) aspects of the practice. But, apparently, in answer to your
question, there are trephynations being performed today in the US.
